Find this FORM: Facility Reported Incident (FRI)

Fill it out and submit it - be as specific as you can, no general comments.

They will not share you name at the facility when they are investigated however they need facts so they can make sure the complaints are valid.

You will get a letter to confirm your complaint has been filed and on record with the Centers for Medicare and Medicaid Services.

This is the First Step - be patient - they are very slow, but documentation has to occur., and this is their process.

Note:

If you need to report something that was not witnessed, for example if your loved one has a bruise and no one saw how it happened, click on the box noting: Injury of Unknown Origin.
